The Release of Information form is used to authorize the Cooperative to release health information to
another individual or group. All fields need to be complete and physically signed to honor your request.

Preferred Drug List
This Preferred Drug List is not inclusive of all drugs available on the formulary.
Rather, it represents an abbreviated version of approved drugs on the formulary.
The Preferred Drug List is currently being updated and will be available
