Types of Advance Directives

Power of Attorney for Health Care

An appointed individual that makes health care decisions for you in the event that you are not capable of making them yourself.

Living Will

Describes your wishes if you are faced with a serious health condition and you are not able to speak with others around you.

You should consider your end of life wishes when making an Advance Directive, which may include:

• Independence
• Quality of Life
