Request An ID Card

To create a temporary ID card that you can print out at home, please fill out the information below. Check the box to have a new ID card mailed to you.

If you have not supplied us with your Social Security Number, you will not be able to create a temporary ID card. Please call a Member Services Advocate at (715) 552-4300 or (888) 203-7770 to request a replacement ID card.
