Online Health Risk Assessment (HRA)

The HRA is a valuable tool available to Group Health Cooperative of Eau Claire (GHC) members. The HRA will tell your health age, identify your health risks and provide personalized ways for you to make healthier life choices.

At the end of the HRA questionnaire, you will be able to print your results. The entire HRA takes approximately 15 minutes to complete.

Having the following information nearby will help save you time and provide you with more accurate results:

• Height
• Weight
• Body measurements
• Body fat percentage
• Blood pressure
• Cholesterol

Please note that you do not need to have all of this information available to take the HRA.

To take the HRA, please return to the GHC homepage and enter your ID and Group numbers to login. The login is located in the upper-left hand corner.
