Your Health Promotion Program Benefit

The GHC Health Promotion Program Benefit is designed to improve the overall quality of life for Eau Claire Area School District (ECASD) employees and their families and, in the process, control future health care costs.

At the beginning of the new plan year in July, employees who achieved the discounted rate for participating in the Health Promotion program benefit for 2010 will remain at the discounted rate. Employees who did not complete the requirements for the Health Promotion program benefit for 2010 will remain at the non-discounted rate.

In order to obtain or maintain the reduced premium, employees and spouses (on the health plan) must complete each of the three steps listed below by October 31st, 2011. Additionally, to earn Eau Claire Chamber Gift Certificates, steps must be completed by September 30th, 2011.

Step 1 - Know Your Numbers

In step 1, ECASD employees, retirees and covered spouses each complete a biometric screening evaluation. This will take approximately 5 minutes. These screenings will include:

  • Height
  • Weight
  • Body mass index
  • Body fat
  • Blood pressure

Step 2 - Know Your Risks
In step 2, ECASD employees, retirees and covered spouses each complete an online health risk assessment. This will take approximately 10-15 minutes. Be sure to have your biometric numbers from Step 1 handy. To take the HRA, please return to the GHC homepage and enter your six-digit GHC Member ID number and Group number including division to login (member ID, group and division numbers can be found on your GHC Member ID Card). The login is located in the upper-left hand corner.
 

Step 3 - Know Your Health
In the last step, ECASD employees, retirees and covered spouses meet individually with a GHC Health Promotion Coach to review the results of steps 1 and 2. This will take approximately 15 minutes.
